ABSTRACT

We report the case of a young woman with generalized dystonia treated using deep-brain stimulation (DBS) guided by intraoperative physiological signals. During the procedure, local field potentials (LFPs) recorded from the internal globus pallidus revealed delta-band activity (2-3 Hz) that coincided with her dystonic symptoms. These electrophysiological signals were used to refine electrode placement and guide postoperative programming. Six months later, her symptoms had markedly improved with no reported complications. This case illustrates the clinical application of LFPs in dystonia and supports the use of intraoperative biomarkers to personalize DBS in movement disorders.
