Chronic hindfoot pain due to calcaneal osteochondroma: magnetic resonance imaging findings

ABSTRACT

Osteochondroma is the most common benign bone tumor; but its location in the calcaneus is extremely rare. We present the case of a 74-year-old woman with chronic plantar pain and hindfoot instability. The radiograph revealed a posterolateral exostosis of the calcaneus. Magnetic resonance imaging (MRI) confirmed a pedunculated lesion, with medullary and cortical continuity, covered by hyaline cartilage and in contact with the fibula brevis tendon, which showed inflammatory changes due to chronic friction. This case highlights the usefulness of MRI for the structural and functional diagnosis of atypical causes of persistent heel pain.
