ABSTRACT

A Grynfelt hernia is a rare abdominal wall defect that protrudes through the superior lumbar triangle. Described by Grynfelt in 1866, it can be congenital or acquired. It represents 1.5-2% of abdominal hernias and typically presents around age 60. It can be asymptomatic or manifest as a painful lumbar mass. Its low incidence makes diagnosis difficult. An 82-year-old woman presented with a 2 × 2 cm Grynfelt-Landis hernia diagnosed by computed tomography scan. Laparoscopic transluminal abdominal hernia repair (TAPP) was performed using a polypropylene mesh. The patient’s recovery was uneventful, and she was discharged the following day. Grynfelt hernias are rare and often go undiagnosed; computed tomography is the diagnostic study of choice. The treatment is always surgical, and preperitoneal laparoscopic mesh repair is a suitable approach with good results. Further research is needed to standardize the treatment.
