Laparoscopic radical cure for defects of the abdominal wall. Results 5 years after

ABSTRACT

Introduction: The laparoscopic approach is, nowadays a valid option for the treatment of ventral hernias.
Methods: Between April, 2002 and September, 2006, 50 patients underwent laparoscopic-assisted surgery of 52 incisional hernias, most of them (48) placed in the umbilical midline. The mean follow-up was 36 months in all cases.
Results: The mean length of hospital stay was 2.4 days. Chronic pain due to the infraumbilical wounds reparation has been referred in 3.8% cases. The overall complication rate was 3.8%, including an inadvertent bowel injury during laparoscopic adhesiolysis which needed re-admission and surgery. Recurrence occurred in one case of subcostal incisional hernia.
Conclusions: Laparoscopic-assisted reparation of ventral hernia is technically feasible and safe in selected patients. It can be associated to pain in relationship with the type of stiches, with a difficult management, or even to serious complications such as bowel perforation that could be avoided by limiting the use of electric energy sources.
