Giant mucocele of the cecal appendix: A case report and brief review

ABSTRACT

Objective: To report a case of a patient with abdominal pain in which colonoscopy examination shows no disturbances on cecal region, computed tomography imaging suggested the presence of a cystic mass belong to cecum, and surgical exploration found a giant mucocele from appendix. Case report: A 72 years-old male was admitted in our service because of mild pain in the right iliac fossa and mild fever, which disappear spontaneously. Physical examination shows a mobile mass in abdomen, well defined like a water filled balloon with no pain nor tenderness under palpation. Computed tomography imaging shows a homogeneous cystic mass, belong to cecum, without infiltrated organs or distant metastasis. Tumoral serum markers CA 19-9 and CEA were negative. A middle laparotomy found an appendicular giant mucocele who sized 9 x 9 x 8 cm. completely resected with free base after a simple appendicectomy. No evidence of perforation or metastasis was detected during surgery. Hystopathological diagnosis was appendicular mucocele with mucinous cistadenoma. Conclusions: Appendiceal mucocele is a rare lesion, with unspecific symptoms or even completely asymptomatic, which difficult a preoperative diagnosis. However, a pre/transoperatively diagnosis is necessary to perform a careful resection of lesions in order to avoid iatrogenic rupture under surgery.
