Neurotization of irreparable and late lesions of the branchial plexus

ABSTRACT

Objective. To determine the results of neurotization in ancient brachial plexus injuries by intercostal nerve grafting to the damaged trunks. Material and methods. A series of 13 patients (11 male and 2 female) aged 3 to 35 years, who had an ancient injury of the brachial plexus trunks and were classified in 2 groups: injury was pre-ganglionar in 2 cases (group I) and post-ganglionar in 11 (group II) is reported. Type A neural fibers of 6 intercostal nerves and sural nerve were transplanted into one or two primary trunks and sutured by 8 or 9-0 nylon. A Velpeau bandage was applied for 3 weeks. Results. After 18 months of follow-up, and a rehabilitation program, gross sensitivity was progressively recovered. Better results were observed in group II patients. Conclusion. Functional improvement can be expected in those patients who have post-ganglionar injuries of the brachial plexus by neurotization of primary trunks.
