Obesity. A weight problem. The current and experienced therapeutic role of sibutramine. The weight of evidence

ABSTRACT

This article reviews the most recent information of therapeutic effect of Sibutramine in overweight and obesity states with and without co-morbidities. Overweight and central obesity, thought adipocyte dysfunction and lipotoxicity, are inducer of insulin resistance and the consequent Diabetes mellitus type 2 and cardiovascular disease, making desirable any strategy directed to control of corporal weight. Sibutramine is a drug which has show efficacy and safety to weight loss in 10 years mainly in clinical trials and postmarketing surveillance. Sibutramine is a neurotransmitter reuptake inhibitor of serotonin and norepinephrine in CNS, specially in hypothalamus. Paracrine increase of neurotransmitters, which are ligands of α1 and β1 adrenergic- receptors and 2A/2C serotonin receptors, causes stimulation and maintenance of satiety. In addition, Sibutramine limits down regulation of basal metabolism associated to weight loss and stimulate thermogenesis of adipocytes via activation of adipocyte β3 peripheral receptors. In long term clinical trials, has been documented mean weight loss of 10.2 kilograms in 24 months with 10 to 15 mg of Sibutramine. And also have global improve in cardiac and metabolic risk markers, specially lipidic and glycemic profile. These favorable effects are well documented of both obesity and overweight associated or not to comorbidity like Diabetes mellitus type 2, hypertension and dyslipidemia. Sibutramine is a good alternative on integral treatment of overweight and obese patients, after 10 year of marketing.
