A model of laparoscopic training for the bile duct via exploration

ABSTRACT

Laparoscopic cholecystectomy is the most frequent laparoscopic procedure in General Surgery. Only few surgeons can perform a transoperative cholangiography or common bile duct exploration during laparoscopic cholecystectomy and that is why recent data shows that ERCP seems to be the new gold standard for diagnosis and treatment of common bile duct stones. Laparoscopic exploration of the common bile duct is considered a demanding procedure, more complex than the laparoscopic cholecystectomy alone, but it offers a minimally invasive approach during the same surgical procedure minimizing the morbidity and mortality risks of an open surgery or postoperative ERCP. Laparoscopic exploration of common bile duct is feasible, simple and even safer when appropriate laparoscopic training is completed. Our experience shows that laparoscopic training during a residency program is more effective than laparoscopy courses alone. This training model for laparoscopic exploration of the common bile duct is a simple, affordable and reproductible method for developing new surgical skills in laparoscopic surgery.
