Association between persistent ductus arteriosus and combined aortic stenosis and insufficiency for functionally bicuspid aortic valve

ABSTRACT

A 32 year old woman was referred with a history of heart failure and chest pain. The physical findings suggested the presence of combined aortic stenosis and insufficiency but did not explain satisfactory the clinical status of the patient. We performed an echocardiographic study which established the diagnosis of persistent ductus arteriosus associated with the presence of a functionally bicuspid aortic valve presenting clinically as a combined aortic valvulopathy. The patient was treated with percutaneous occlusion of the ductus arteriosus with an Amplatzer type occluder dispositive, obtaining a remarkable improvement of her functional status without the need of immediate surgical management, in spite of the association with the aortic valvulopathy. The diagnosis of this association was confirmed with the echocardiographic and angiographic studies, since clinic and radiologically it showed characteristics of an aortic valve affection. The association between a combined aortic valvulopathy secondary to a fusion of one of it’s commisures with a persistent ductus arteriosus has not been reported previously in the literature. The interventional management was preferred over the surgical one since the aortic valvulopathy didn’t fulfill surgical criteria that would have justified the operative procedure for both pathologies.
