Effective doses of rocuronium for satisfactory intubation in children under age two submitted to surgery

ABSTRACT

Background: Rocuronium bromide is a non-depolarizing muscle relaxant with a short beginning of action, wide security margin and hemodynamic stability, but it is not regularly used in our hospital in children under 24 months. Objectives: (1) Finding the adequate doses of rocuronium for tracheal intubation in children under age two; (2) comparing the optimal dose with those reported in literature for children over age two; and (3) assessing the optimal time to orotracheal intubation with the various doses. Material and methods: This was a prospective, comparative, longitudinal trial in 30 children under 24 months, ASA I-II, scheduled for elective surgery shorter than two hours requiring general anesthesia. Ten children were assigned randomized into each of three groups, given 0.3 mg/kg, 0.6 mg/kg and 0.9 mg/kg rocuronium, respectively. Patients were intubated 60 seconds after giving the muscle relaxant. All data were recorded in a data collection sheet. Results: Intubation conditions were better in the groups receiving 0.3 mg/kg and 0.6 mg/kg rocuronium, whereas in two patients in the group with 0.9 mg/kg reversion of the relaxant was necessary. Patients remained hemodynamically stable. Conclusion: The optimal dose of rocuronium for intubation in children younger than 24 months was 0.6 mg/kg.
