Interventionism treatment in diabetic patients with unstable angina: Stent vs ballon

ABSTRACT
The purpose of the current study is a retrospective analysis of an hospital and long term outcomes following stent implantation in native coronary arteries in diabetic patients with unstable angina.
Methods: Study Population. The patient cohort includes a consecutives series of 2,841 in the Hemodynamic Angioplasty Database between October 1995 and December 2003. The diabetic patients with unstable angina were divided into two groups: 1. diabetic patients treated with stent and 2. diabetic patients treated with balloon angioplasty.
Results: We studied 2,841 consecutives patients into of this 263 diabetic patients with unstable angina 145 patients treated with stent and 118 patients treated with balloon angioplasty. Overall, demographic in patients with stent or no stent were similar. Patients treated with balloon had more a previous infarct and post-infarct angina (p ‹ 0.05 compared with the other group). In quantitative angiographic results showed that the reference vessel size, diameter stenosis and lesion length was significantly bigger in patients treated with stent (p ‹ 0.05 compared with other group). Postintervention, the minimal lumen diameter were significantly bigger (p ‹ 0.05) in patients treated with stent (3.13 ± 1.30 mm) compared with patients treated with balloon (2.28 ± 0.12 mm). The patients treated with balloon had a significantly lower (p = 0.030) successful angioplasty rate (90.67%), compared with patients treated with stent (97.24%). Clinical outcome, there were no differences in the frequency of adverse cardiovascular events between the two groups during intra-hospital evolution. The incidence of recurrent angina (56.41% vs 27.17%) were significantly bigger (p ‹ 0.05) in diabetic patients treated with balloon. The incidence of event-free survival was significantly lower (58.63% vs. 39.83 % with p ‹‹ 0.05) in this group.
Conclusions: The coronary stent implantation was associated with significantly bigger minimal lumen diameter and successful angioplasty rate. This was associated with significantly lower incidence of recurrent angina and better event free survival during the first 6 months.
