Risk-benefits of the routine transoperatory cholangiography

ABSTRACT

Mirizzi introduced the transoperatory cholangiography initially adopted in most of the procedures (80-85%). Today it is only done in 6% of all cholecystectomies. During the laparoscopic era, the number of bile duct lesions increased importantly and intraoperative cholangiography could reduce this kind of lesions. Objective: To identify the risks and demonstrate the benefit of routine transoperatory cholangiography. Material and methods: Five year retrospective study, in the ABC Medical Center, in Mexico city. We included all patients that went to cholecystectomy. Results: We included 1257 patients who went to cholecystectomy. 95% of laparoscopic procedures. Only 14% of cholangiographies done. 81% of normal cholangiographies, 5.6% of choledocholithiasis, and 1.7% of anatomic variants. Conclusions: Transoperatory cholangiography is a secure procedure that in expert hands doesn´t take much time, it detects anatomic variants, and bile duct stones not suspected preoperatory. Transoperatory cholangiography up to the surgeon could be done as a routine procedure.
