Tertiary evaluation for the donor heart transplantation

ABSTRACT

Objective: To present the experience of our group in tertiary evaluation of donor hearts. Design: Consecutive cases. Location: Different authorized centers for organ donation in the country. Patients and participants: We present the experience of the tertiary evaluation in potential donors realized between January 1, 1993 and December 31, 2007 and the results of this direct evaluation of the hearts realized thru median sternotomy, direct visualization and palpation of the hearts and near structures. Results: In this time period we made 23 tertiary evaluations, and based on the direct inspection of the hearts we eliminated three cases for harvesting: one for size disproportion donor-receptor, other in which we found an hiliar tumor with increase in number and size of periaortic lymph nodes, and the last one, harvested by other team outside of the city, with diffuse hemorrhagic areas and multiple infarcts confirmed in the histopathological evaluation. The other 20 cases were successfully transplanted. Conclusion: It was concluded that direct evaluation of the hearts is relevant for the success in surgical procedure of harvesting and transplantation.
