Cálculo de lente intraocular en pacientes con cirugía refractiva previa

ABSTRACT

Objective: To evaluate the refractive exactitude of intraocular lens power calculation in patients with history of refractive surgery.
Method: The files of 5 patients (6 eyes) with refractive surgery history and facoemulsificación were reviewed. The keratometric data were registered (K), by axial topography (Humphrey) and Orbscan, K average, flatter K, keratometric power at 3 and 5 mm and Total Mean Power at 2 mm (TMP 2 mm).
The calculation of the IOL was made with each of these parameters and 4 formulas: SRK II, SRK-T, Holladay and Hoffer.
The postoperative spherical equivalent was calculated and the power of the implanted IOL was compared with the results obtained with the other formulas and K. With these results we analyzed which IOL power would have been the best.
Results: The flattest values of the K corresponded, to the lowest K obtained by Orbscan and TMP 2 mm. We obtained the most suitable IOL power with the former K and the SRK-T formula. In general the values obtained by TMP 2 mm, tend to be higher than those required for emetropia.
Conclusions: The flattest K with Orbscan and SRK-T is the best combination, nevertheless on it’s own it provides a general direction for choosing a correct IOL. It does not apply for corneal ectasia.
