Surgical laparoscopy of colon and rectum. Initial experience at the Hospital General de México

ABSTRACT

Introduction: Laparoscopic colorectal surgery has had a slow acceptance in the treatment of benign and malignant conditions; the procedures are technically more difficult, the costs are greater, an increased operating time and concerns regarding compromised oncologic outcomes. Objective: Describe the initial experience of laparoscopic colorectal surgery in the Coloproctology Unit in the Gastroenterology Service in the General Hospital in Mexico City. Material and methods: Observational, retrospective, descriptive study, of patients with laparoscopic colorectal surgery from December 2006 to July 2007. Results: Fourteen patients were evaluated, 9 males and 5 females, the median age was 60.3 years (range was 21 to 80 years). The preoperative diagnoses included 3 patients with benign diseases and 11 patients with malignant conditions. The median operation time was 275 min (range 120 to 360 min) and the estimated blood loss was 236 mL. The median time to resumption of a regular diet was 3 days and the length of hospital stay was 7 days. The postoperative morbidity rate was 15% and there was one dead secondary to acute renal failure. Conclusions: The laparoscopic approach is feasible to perform for most benign and malignant conditions, without conversion to conventional surgery and low complication rates; it is also associated with less blood loss, reduced pain, quicker recovery and shorter hospital stay.
