ABSTRACT
Meconium inhibits the surface tension lowering the properties of surfactant, because of this mechanism, in neonates with this problem the treatment with surfactant might be beneficial. The case of newborn with a meconium aspiration syndrome and severe persistent pulmonary hypertension, who was treated with conventional mechanical ventilation and surfactant therapy in early stages, is presented. He showed important improvement whit this management, and did not need high frequency ventilation or inhaled nitric oxide.
