Training of the bariatric surgery

ABSTRACT

With the ever growing obesity epidemic, formation of human resources is today more important than ever, especially in developing countries such as ours. Any of the procedures for the treatment of morbid obesity require of formal training to optimize results during the learning curve, minimizing morbidity and mortality. The learning curve for any of the bariatric surgery procedures has been established to be between 50 and 75 cases. To this day, three types of training in bariatric surgery are available: 1) the weekend courses, with a 3 day duration no practice in patients, with occasional animal lab exposure; 2) the minifellow, a course that can last up to six months, of interest for experienced laparoscopic surgeons; with the possibility to integrate the multidisciplinary team that will eventually take charge of the patient before and after surgery. This course may include limited hands-on practice and 3) the formal fellow training program, lasting a minimum of one year, where the surgeon in training participates actively in the perioperative management of the patient, with hands on training and the opportunity to develop his or her learning curve in a supervised academic environment with an experienced surgeon. Published evidence suggests that the latter form of training prepares best the surgeon in training in this specialty, especially if this training occurs in a center where more than 100 cases are operated on each year; a factor known to reduce morbidity and mortality in this group of patients.
