ABSTRACT

Apnea, defined as cessation of breathing resulting in pathological changes in heart rate and oxygen saturation. Although apnea typically results from immaturity of the respiratory control system, it also may be the sign of other diseases or pathologic states that frequently affect preterm infants. This unstable breathing may lead to severe cerebral hypoxia and be life threatening, especially in preterm infants. Many questions on the mechanisms responsible for controlling neonatal apneas remain unresolved. Secondary causes of apnea should be excluded before a diagnosis of apnea of prematurity is made. Methylxanthines and Continuous Positive Airway Pressure are the mainstay treatment of apnea in neonates. Mechanical ventilation is reserved for apnea resistant to above therapy. An update to the management of apnea in neonates has been described in the present review.
