Laparoscopic treatment of the simple hepatic cysts

ABSTRACT
Introduction: The simple non parasitic hepatic cysts have a global incidence of 5%. Commonly they are located in the right lobe, they are diagnosed incidentally during radiological exams of routine. The symptomatic cysts are of great size and they are associated with pain, nausea, vomit, gastric fullness, portal hypertension, obstructive jaundice and occasionally thrombosis of portal vein.
Objective: Demonstrate the effectiveness of the laparoscopic treatment.
Material and methods: We revised the cases of non parasitic hepatic cysts treated by laparoscopic surgery during a period of 5 years. The parameters estudied included were: age, sex, symptomatology, radiological studies, complications and recurrence. It was carried out a retrospective observational analysis.
Results: Twenty one patients were treated, 15 women (71.5%) and 6 men (28.5%), with average age of 62 year-old. The most frequent symptoms were pain in right hypochondrium in 15 patients (71.5%), gastroesophageal reflux and pain in 2 (9.5%), gastroesophageal reflux in 1 (4.75%), pain and palpable mass in 2 (9.5%), pain and jaundice in 1 (4.75%). In 18 patients unroofing of the cyst were carried out (85.7%), marsupialization in 3 (14.3%), with 2 to 3 days of intrahospitalary stay. After the surgical treatment there was remission of the symptoms in 20 patients (95.25%), 1 present refilling after the unroofing.
Conclusion: The laparoscopic treatment is effective and safe. In our series we have only one recurrence, so we believe that laparoscopic treatment with unroofing is the best technique for the resolution of this pathology.
