Evaluation of duration and results in replacement of biological heart valve prostheses

ABSTRACT
Background and objective: An option for replacement of diseased heart valves are the bioprosthesis. However their longevity makes mandatory the replacement of the bioprosthesis with an increase in morbidity. This article shows the durability of the bioprosthesis and the results of their replacement when they fails.
Material and methods: From January 1, 1996 to October 31, 2000, 50 patients (12 males and 38 females), with a mean age of 48 years old were submitted to replacement of deteriorated bioprosthesis evaluated by clinical and echocardiographic studies. It were analyzed the longevity of the biological prosthesis, and morbidity and mortality during their replacement.
Results: It’s were realized 44 mitral, 10 tricuspid, 9 aortic and one pulmonary bioprosthesis replacement. The ischemia and cardiopulmonary bypass times were 76.5 and 122.8 minutes. Seven patients were complicated with renal failure, excessive bleeding, brain oedema, pericardial effusion or sepsis. Five patients died caused for cardiogenic shock and four cases and septic shock in one patients. The mean time of use for the bioprosthesis before their failure was 12 years.
Conclusion: It was concluded that with a well established indication, the use of bioprosthesis has a useful longevity and the results of their replacement in our experience are similar to the medical literature information.
