Urgent surgery in complicated coronary angioplasty

ABSTRACT

Background: The increase in the number of percutaneous coronary angioplasty and the higher risk of surgery when the angioplasty is complicated, we present the frequency and results of this procedure. Material and methods: From September 1, 1998 to august 31, 1999, 411 percutaneous coronary angioplasty was made. Urgent cardiac surgery was indicated in eight patients (6 male and 2 female), due to complications of the procedure. The mean age was 59.3 years, range of 35 to 72 years old. Results: In seven patients the internal mammary artery was employed for surgical revascularization and in six cases the intraaortic balloon pumping was used. The time between accident and surgery was 4.3 hours ranging 1 to 16 hours. It was 3 perioperative myocardial infarctions, one patient was reopened for excessive bleeding and this case developed a mediastinitis. One patient presented severe brain damage and 2 patients died due to refractory myocardial failure. The mean of in hospital length of stay was 32.5 days ranging 7 to 63 days. Conclusion: lt was concluded that despite a lower incidence for urgent cardiac surgery for complicating coronary angioplasty, the morbidity, mortality and length of stay increases. On the other side surgical procedures offers a complete revascularization.
