Laparoscopic Treatment of Complicated Paraduodenal Hernia

ABSTRACT

Paraduodenal hernias are a rare entity that represents 30 to 53% of all the internal hernias. The aim of this manuscript is to present the case of a 55-year-old female, with multiple abdominal surgeries, Glanzmann’s thrombasthenia, with intestinal occlusion that didn’t resolve with medical treatment. A diagnostic laparoscopy was performed which demonstrated a paraduodenal hernia; intestinal liberation and closing of the hernial orifice was performed. Fifteen days later she presented again with intestinal occlusion. The upper gastrointestinal contrast films showed an incomplete obstruction in the fourth portion of the duodenum, in spite of the resolution of the hernia, which did not responded to the conservative treatment; a new laparoscopy was performed in which an intense fibrosis process was observed at this level, being difficult to liberate specially because of the adhesion to the superior mesenteric artery. A laparoscopic Roux in Y gastro-jejunal anastomosis was performed and she was discharged without complications. We reviewed the literature of this uncommon pathology.
