ABSTRACT

Background: Hand involvement is a salient feature of rheumatoid arthritis (RA). Despite thermo therapy is frequently used for treating RA hand involvement, the evidence about its efficacy for this condition is remarkably scarce. Objective: To compare the efficacy of two modalities of thermo therapy: fluid therapy (FT) versus therapeutic ultrasound (TUS) for treating hand involvement in RA. Material and methods: Thirty RA patients (48.5 ± 12.5 years old; 12.5 ± 10.8 years of disease duration) were randomized to FT (15) or US (15). In an open basis, pain intensity in addition to global and hand-specific functional status were assessed at baseline, and after 5, 10, and 30 days after completing physiotherapy. Results: No intergroup differences were noted on functional status. Subjects randomized to TUS had lesser pain intensity only at 10 days (p = 0.04). Compared to baseline, a progressive and significant improvement was noted for pain intensity, and functional status in the whole group. Conclusions: Thermo therapy, delivered either by FT or TUS, seems to be efficacious as an adjunct treatment for improving pain intensity and functional status in RA hand involvement.
