Parapagus dicephalic conjoined twins. A case report and literature review

ABSTRACT

Conjoined twins, commonly named siamese twins, are infrequently seen with an incidence of one case in 100,000 live births. Classification depends on the points of non-separation or fusion. Those cases joined side by side are called parapagus and can have two heads (dicephalic) and two thoraxes (dithoracic). A case of parapagus, dicephalic conjoined twins is described. Diagnosis was established by prenatal ultrasound at the 23rd gestational week. The study showed conjoined twins with two heads, two separate necks, four arms, fused thorax at the level of the nipples, single heart with atrioventricular concordance, ventriculoarterial discordance, 2 atria, left ventricular isomerism, double aorta. There were only two lungs and shared abdominal organs, female genitals and only two legs. Pregnancy was ended at the 29th week, the product only survived for 5 hours due to the complex cardiopathy. A review of literature and discussion and emphasis on prenatal diagnosis and the ethical considerations are made.
