Laparoscopic splenectomy with harmonic scalpel for hematologic diseases. Ten year experience in a private hospital

ABSTRACT
Introduction: We present 10 years of experience with LS at a private hospital, all of them with pedicle dissection with harmonic scalpel and individual staples for the hemostasis of vascular pedicle.
Methods: This is a retrospective review of LS in patients with hematologic diseases at Hospital Angeles Lomas, Mexico. All cases were studied and referred by the hematologist, in a ten year period. In all of them, harmonic scalpel and individual staples were utilized for the dissection and hemostasis of the vascular pedicle. We report indications for surgery, operating time, surgical complications, length of hospital stay and post operative follow up.
Results: In this period, 19 patients, average age 34.9 years, were referred to surgery by the hematologist. Two cases required cholecystectomy as well. All patients were operated by laparoscopy. Average operating time was 192 minutes. There were no operative complications, nor mortality in this series. Average hospital stay was 3.6 days. All patients achieved remission of their hematologic disease.
Conclusions: The laparoscopic approach for splenectomy is feasible, safe and very convenient for the benefits to the patients. The technique we describe has an attractive cost – benefit compared to the use of automatic staplers.
