Bazin’s indurated erythema. Report of a pediatric case

ABSTRACT

Tuberculosis is a worldwide public health problem. Cutaneous tuberculosis is one of its extrapulmonary manifestations. Bazin's indurated erythema is characterized by a tuberculid which is a chronic indurated lesion with a tendency to ulcerate. It usually appears in the lower limbs; it is more common in females, in winter time and uncommon in childhood. We present the case of a 10 year old girl with erythematous nodular lesions and non-painful ulcers in the lower limbs of one year of evolution. It did not respond to a treatment with topical antiseptics. A Bazin's indurated erythema was the suspected clinical diagnosis. It was corroborated by the presence of PPD hiperergia, a chain reaction in situ, polymerase positive for M. tuberculosis and an excellent response to a treatment with four antituberculosis drugs. We present this case and review of the literature, given its low frequency in children as well as the difficulty in diagnosis. A precise correlation of clinical with pathological findings and PPD test and a response to treatment are essential given the fact that cultures are generally negative.
