Posterior brachial flap. Anatomical study and clinical application

ABSTRACT

Posterior brachial flap is classified as axial direct cutaneous based on the cutaneous brachial artery. In 71% of cases it is originated from the medial segment of the brachial artery and 23% of the brachial deep artery. A descriptive, prospective, longitudinal two-phase study is presented: a description in fresh corpses with 50 dissections. The posterior brachial artery was identified and length, external diameter measures, source and route were taken. In the same way we proceeded with comitant veins and the posterior brachial cutaneous nerve. In the second phase two microsurgical flaps were made to cover the elbow and hand, and one local flap for the reconstruction of the armpit. In 100% of the flaps the posterior cutaneous brachial artery was found in the flap axis; its origin was the humeral artery at 56%; deep brachial artery 30%; bifurcation deep brachial artery and humeral 10%; subscapularis artery 2%; segmental vessels deep brachial artery 2%. The length of the vascular pedicle is from 6 to 14 cm; in 100% cases one or two comitant veins were identified.
