ABSTRACT

We inform the case of a 14 year-old male patient, carrier of chronic renal insufficiency, awaiting kidney transplantation, in substitutive therapy with peritoneal dialysis and with previous history of placement of multiple hemodialysis catheters in the subclavian and jugular vessels; who presented with severe superior vena cava syndrome, with important respiratory compromise and disturbances in the state of consciousness. Flebographic studies showed total obstruction of superior vena cava, innominate vein and bilateral obstruction of axillary, subclavian and internal jugular veins. Successful surgical treatment was performed with a veno-venous bypass form the left internal jugular vein at the base of the head, to the right atrium. We inform the technique as a therapeutic alternative.
