ABSTRACT

Idiopathic cystic dilatation of the biliary tree (choledochal cysts) is a rare pathology that is present in 1 of every 100,000 newborns and is characterized by intra or extrahepatic dilatation of the biliary tree. Although etiology is not well known, there are 2 theories about its origin, congenital and acquired. They can present as pancreatitis, cholangitis or choledocholithiasis. Treatment consists of complete surgical resection because up to 20% can become malignant. Case report: 29 year old female patient with a history of 10 years of abdominal pain, early satiety, and abdominal growth. A giant choledochal cyst 35 cm in diameter was diagnosed and was later completely surgically removed. Conclusions: Choledochal cysts are an uncommon pathology. The treatment foundation is complete surgical removal of the cyst, including all the mucosa, to avoid a possible malignant transformation.
