ABSTRACT

Patient is a 41-year-old man, single, workman, from Veracruz, Mexico. He had no important history but from birth presented with scrotal volume increase due to vascular malformation which continued to develop, presenting with spontaneous hemorrhaging that was initially treated with sclerotherapy. Physical examination revealed an uncircumcised penis with prepuce that was difficult to retract, irregular increased volume at scrotal level ventral to the penis that became larger when the patient was standing in upright position due to vascular malformation of bluish hue. No thrill was palpated.
Phlebangiomas are rare congenital vascular malformations that are present at birth. Their growth is evident during puberty. There was esthetic improvement of the genital region after scrotectomy and autologous graft placement and total resection of the vascular malformation at that level. Different treatment methods have been described in the literature. Lesion localization and extension in the present case made its treatment controversial, taking into account the components of esthetics and fertility.
