IgA Multiple mieloma with craniofacial manifestation: A clinical case report and review of the literature

ABSTRACT

Multiple myeloma (MM) is a type of monoclonal neoplasm of multifocal and malignant plasma cells that accumulate in bone marrow. Proper diagnosis should include laboratory tests on blood and urine; bone X-rays and bone marrow biopsy are usually taken also. Identifying abnormal immunoglobulin in the blood and/or urine can help determine whether a plasma cell tumor is present. MM is usually fatal, however certain risk factors can affect the prognosis, such as age, lesion site, renal involvement and histological findings. Once multiple myeloma is found and staged, treatment options may include chemotherapy, radiotherapy, bone marrow transplant and other complementary therapeutic measures. Here we report the case of a 77-year-old man who was referred for a full denture replacement; the patient displayed signs of pain and paresthesia during the intra-oral examination, both of which are associated with an increase in the volume of the right retromolar mandibular and jugal region. X-rays showed “punched-out” bone lesions caused by the tumor. The diagnosis of MM was confirmed based on the presence of mandibular plasmacytoma (borne out by the histopathology report), the appearance of bone lesions limited to the craniofacial bones (revealed by tomography) and monoclonal gammopathy (IgA).
