ABSTRACT

Background: About 25-40% of renal cell carcinoma (RCC), present with distant metastasis at diagnosis. The common metastasis sites are lung, bone, liver and brain. Penile metastases are extremely uncommon.
Objective: Report a case of renal cell carcinoma with penile metastasis as initial manifestation.
Case: 66 year-old male, presented with a tumoral lesion in the penis. At the physical examination a 3 mm nodular lesion protruding the urethral meatus was found, indurated glans, and mild pain. The histological examination reports RCC. The tomography revealed a 14 cm left renal mass, heterodense, with Gerota’s fascia invasion and widespread lymph node involvement. It was proposed palliative surgical approach; the patient didn’t accept it, dying 8 weeks later.
Conclusion: The penile metastases from RCC are very unusual and in the reported case this metastasis as initial manifestation implied a poor prognosis.
