Xantogranulomatous pyelonephritis with perirenal abscess extended to the abdominal wall in a pregnant woman

ABSTRACT

Xanthogranulomatous pyelonephritis is a chronic bacterial infection of the kidney, characterized by the replacement of normal renal parenchyma with lipid-loaded macrophages. A direct relation between pregnancy and xanthogranulomatous pyelonephritis is rare. The case of a 23-year-old woman in her third trimester of pregnancy is presented. She was referred to our institution with a presumed diagnosis of emphysematous pyelonephritis and right paraspinal abscess. Pyelic lithiasis and right emphysematous pyelonephritis with perirenal abscess extended to the abdominal wall was diagnosed by computerized axial tomography. After these findings, she underwent a cesarean section, open drainage of the abscess and nephrectomy. The final histopathological report was xanthogranulomatous pyelonephritis The patient’s condition deteriorated rapidly and she developed a duodenal fistula leading to death from sepsis that was refractory to treatment. The medical literature on xanthogranulomatous pyelonephritis and pregnancy was reviewed.
