Accessory spinal nerve injury. A case presentation

ABSTRACT
Introduction: Injury to the spinal accessory nerve leads to trapezius palsy. The trapezius is a major scapular stabilizer, it contributes to scapulothoracic rhythm by elevating, rotating, and retracting of the scapula. The superficial course of the spinal accessory nerve in the posterior cervical triangle makes it susceptible to injury. Iatrogenic injury to the nerve after a surgical procedure is one of the most common causes of trapezius palsy.
Case presentation: 34 years old female, she had a surgery on left supraclavicaular region in zone II of posterior triangle neck 4 years ago. Presenting pain in shoulder with impossibility of flexion and abduction of left shoulder over 90°; she was treated by empirical without results, she comes to rehabilitation asking about a better shoulder function, clinical diagnosis was confirmed with electromyography and conduction velocity; due a large time evolution, it was proposed the Eden-Lange surgery.
Conclusion: If the injury is diagnosed within 1 year of the injury, microsurgical reconstruction or neurolysis must be considered. Eden-Lange surgical procedure corrects deformity, and improves function in patients with irreparable injury to the spinal accessory nerve.
