ABSTRACT

This paper reports the case of twins born in the hospital after the mother was sent to care in Maternal Fetal Medicine by the presence of one acardic twin, confirmed by obstetric sonogram. At 26 weeks of gestational age, under peridural blocking, through operation caesarean section, two female products were born, the first twin with weight of 990 g, qualified with 3/5 Apgar who required advanced resuscitation maneuvers, admitted at neonatal intensive care unit in critical conditions, started ventilation with high parameters and FiO2 100% survived only 70 minutes, 6 minutes later extracted twin II, acardic, 1,730 grams weight. Cases of fetus or acardic twin, also called twin reversal arterial perfusion sequence (TRAPS) are a rather rare phenomenon, presented with an incidence of 1:35,000 pregnancies or 1% of monochorionic twin pregnancies, the surviving twin ("pump twin") has a high mortality (50-70%) and the acardic twin, different forms of presentation. Here we report the case of the twins, the evolution of pregnancy and the postmortem studies results.
