Gastrointestinal stromal Tumor in paraesophageal hernia: A case report and literature review

ABSTRACT
Introduction: Gastrointestinal stromal tumor associated with paraesophageal hernia, is not common. Both entities are mostly asymptomatic and discovered accidently during the endoscopic or X ray evaluation. Case report: We report a case where both entities were found: an 82 year old man with disphageal symptoms was diagnosed by Computed Tomography and Esophagogram with paraesophageal hernia. He was submitted to laparoscopic surgery for hernia repair. During surgery a gastric wall tumor was detected. Transurgery biopsies reported GIST tumor. Partial gastrectomy was performed with closure of diaphragmatic hiatus. Considering the tumor size and the mitotic count, it was a high risk tumor. The patient outcome has being as expected, taking imatinib with good tolerance. Conclusions: The mainstay of treatment for paraesophageal hernia as well as for GIST remains surgery, laparoscopic approach being chosen, because of decreased postoperative morbility, superior visualization of the hiatus, better tumor approach and minimum tumor manipulation.
