ABSTRACT

This case report presents the endodontic treatment of a mandibular second molar with four root canals, three of which were located in the mesial root.
A third middle mesial canal was found between the mesiobuccal and the mesiolingual.
The canals were shaped using ProTaper Universal rotary files and irrigated using 2.5% sodium hypochlorite, 17% EDTA and normal saline solution as the final irrigant. The canals were then obturated using a single ProTaper Universal gutta-percha cone and Sealapex sealer.
This case shows a very rarely found anatomic configuration and highlights the importance of having a knowledge of internal root canal anatomy in order to minimize any possibility of failure.
