The use of OK-432 (Picibanil) as a non-surgical alternative for the management of ranula and mucoceles

ABSTRACT
Ranula and mucocele are clinical terms used to describe a pseudocyst associated with mucosal extravasation in the surrounding tissue wherever minor salivary glands are found. These lesions occur as a result of trauma to the excretory duct of the salivary gland or due to an obstruction of the saliva flow caused by an associated lesion, such as a sialolith or bacterial plug. These pathologies originate in a minor salivary gland and are also known as mucosal retention phenomena or mucosal escape response.
In all cases, treatment of the lesion involves surgery; however, some surgery-related difficulties, postoperative complications, and permanent consequences in the patient do occur. At the present time, such lesions are managed using other treatment options, such as sclerotherapy, botulinum toxin type A, la ser, or OK-432 (Picibanil), which is a mix of a low-virulence strain of Group A streptococcus (streptococcus pyogenes), and penicillin G in lyophilized powder, which functions as a sclerosing agent in lymphangiomas and as an antineoplastic agent.
We present a review of the literature dealing with this pathology and a new treatment option using this drug, its indications and contraindications, its administration, side effects and complications during the management of reactive lesions associated with minor salivary glands.
