Atypical peritoneal tuberculosis in a cirrhotic patient with laparoscopic diagnosis. A case report and literature review

ABSTRACT

Background: The atypical tuberculosis is a challenge to establish the diagnosis even in our era; is an endemic and chronic disease, own communities with malnutrition and immune deficiency. It is a global public health problem, as it occupies the second leading cause of death among infectious diseases. Case report: Female 43, multigravidas, with exposure to wood smoke for 18 years; chronic hypertension over 20 years of evolution controlled with propranolol, enters the Internal Medicine Department with a diagnosis of pleural effusion; during the study protocol was discovered that gallstones at laparoscopic cholecystectomy is subject to electively, finding extensive macronodular cirrhosis and multiple adhesions of image «grain mijo» which were diffuse across the peritoneum; the pathology report atypical peritoneal tuberculosis. Results: Although intentional search of acid-alcohol bacilli, was necessary to perform laparoscopic biopsy to identify. The patient received antituberculous medical treatment, progressing satisfactorily so far. Conclusions: Tuberculosis remains a contagious disease of high morbidity and mortality, which may require invasive procedures for diagnosis. Laparoscopy is an effective and economical method complementary to the protocol of these patients.
