Single-port laparoscopic hysteropexia: a choice in gynecological surgery

ABSTRACT

Objective: To present a laparoscopic hysteropexy (uterine ventrofixation) technique with a single port, as an alternative to laparoscopic hysterosacropexy. Setting: Private Hospital, Poza Rica, Veracruz, Mexico. Design: Case reports. Method: Two patients with uterine prolapse underwent laparoscopic uterine ventrofixation with a single port approach. A laparoscope with a working channel was used. Laparoscopic instruments included 5 mm x 43 cm needle holders, needle bending instrument, knot pusher, Kelly clamp, Craw forceps, hook-needle, passing suture needle and there was assistance with percutaneous needle and endosuture. Results: The operative time for each case was 40 and 45 minutes. Patients were discharged within 24 hours. There was a follow-up of 10 years and 12 months respectively. No complications were reported, patients were asymptomatic and without recurrent uterine prolapse. Conclusion: Laparoscopic uterine ventrofixation surgery with a single port is an alternative treatment for uterine prolapse when sacropexy or sacrospinous fixation are contraindicated. The procedure has a short operating time, a quick recovery and good cosmetic results. More cases and further studies are needed to give statistical validity to this procedure.
