Laparoscopic transabdominal preperitoneal inguinal plasty without fixation

ABSTRACT
Background: At a world level, laparoscopic inguinal plasty only presents a %5 acceptance when compared with the 95% of cholecystectomy. In our country, the percentages are similar to the public and private hospital percentages from urban areas. Objective: To present our initial experience in the technique of laparoscopic trans-abdominal pre-peritoneal inguinal plasty without fixation. Material and methods: It was recorded the results from the patients operated between June, 2009, and October, 2011. The sample was composed by 44 patients, who were performed laparoscopic trans-abdominal pre-peritoneal inguinal plasty without fixation of the polyester prosthetic material that was anatomically preformed and designed for the left or the right sides, measuring 10 x 15 centimeters. The studied variables were as follows: gender, age, evolution time, localization, clinical presentation, comorbidities, complications and hospital stay. Results: From the 44 patients, 37 were from the male gender, and 7 from the female gender, with ages ranging from 20 to 75 years old; evolution time from 5 to 75 months; 16 left inguinal hernias, 15 right inguinal hernias, and 13 bilateral hernias; 40 hernias were programmed and 4 of them were emergency cases; 27 patients were addicted to alcohol, and 24 of them to tobacco; there were 13 without comorbidity, 6 with arterial hypertension, 1 suffering from uncontrolled diabetes mellitus, and 1 with prostatic hyperplasia. There was an inadvertent bladder lesion: thirty-nine patients remained hospitalized one day, three patients two days, and one patient four days. Conclusions: Laparoscopic trans-abdominal pre-peritoneal inguinal plasty may be carried out without any specific fixation; and it avoids nervous and vascular lesions with the benefits of laparoscopic surgery.REFERENCES
