Lessons learned from a 20-year experience in laparoscopic transabdominal preperitoneal (TAPP) hernioplasty

ABSTRACT
Introduction: Even though Ger reported the first laparoscopic inguinal hernioplasty, it was not until the decade of 1990 when the new technology of minimally invasive surgery (MIS) was adopted, and new techniques were developed. The two techniques which keep on being performed with the same effectiveness are the trans-abdominal pre-peritoneal (TAPP) technique and the totally extra-peritoneal (TEP) technique. From both techniques, our group has practically adopted the first technique for all the cases since 1991, as it is the ideal procedure for most of the patients. Since then, we have practiced it with modifications, according to our experience learned along these two last decades.
Objectives: To expose some of the most relevant points in the learning of the technique of type TAPP laparoscopic hernioplasty and from the changes that we have adopted along this time.
Methods: Description and discussion of the evolution that the technique of laparoscopic trans-abdominal pre-peritoneal hernioplasty has presented according to the experience of our surgical group.
Conclusions: The TAPP procedure, which we have been performing since almost twenty years ago, has experienced certain modifications regarding subtle technical details, which are the result of life experiences and technological improvements. Nowadays, we may assert that the aforementioned technique is appropriate for most of the cases requiring inguinal hernioplasty, a technique that surely will continue to be improved along time.
