Spiegel hernia: a case report and literature review

ABSTRACT

Background: The Spiegel hernia is an uncommon type of ventral hernia in which there is protrusion of the abdominal contents through a defect in the Spiegelian aponeurosis. It has a variable clinical presentation and the clinician must have high level of suspicion to make the diagnosis. The treatment is surgical, with the possibility of performing either a conventional vs a laparoscopic approach. Clinical case: presentation 52-year-old female patient who presents with a mass in the lateral aspect of her abdomen, which has gradually increased in size during the previous three years. She was studied with a computed tomography that revealed a large Spiegel Hernia. The defect was repaired successfully laparoscopically with the application of a mesh. Discussion: When performed by a skilled laparoscopic surgeon, the laparoscopic repair of the Spiegelian hernia is an effective method both for the diagnosis and management of the disease. It favors a prompt recovery, with a low recurrence rate, and permits the assessment of viability in case of a strangulated hernia. Conclusion: The laparoscopic treatment of the Spiegel hernia has multiple advantages over the conventional approach, which is why it can be considered as the method of choice for the treatment of this disease.
