De Garengeot hernia. A case report and literature review

ABSTRACT
Objective: To report a case of a de Garengeot hernia.
Setting: Third Level Health Care Center.
Design: Case report.
Case description: We present the case of an 82-year-old woman, admitted with a diagnosis of high intestinal occlusion due to a probable complicated inguinal hernia. For this reason she was subjected to an exploratory laparotomy that revealed a Richter’s hernia and a de Garengeot’s hernia in the left femoral canal. These were treated with an appendicectomy, reduction of Richter’s hernia, and closure of the femoral canal with non-absorbable suture. The patient evolved satisfactorily and is currently without complications.
Discussion: The presence of the herniated cecal appendix in the femoral canal is known as de Garengeot’s hernia. Its frequency is very low, preoperative diagnosis is very complex and treatment is related with the evolution time and coexistence of the inflammatory process. Appendicectomy is recommended in the presence of complicated appendicitis, closing the hernia defect with suture without using prosthetic materials.
