Traditional inguinal hernioplasty in the laparoscopic age. Analysis of a private and a public hospital

ABSTRACT

Inguinal hernia surgery is the operation most commonly performed by a general surgeon. Due to its frequency and associated costs, it constitutes a socio-economic problem. There are many techniques to perform the operation. This study was undertaken to analyze the best method to solve this problem. It is a retrospective analysis of all cases of hernia surgery done over a period of one year at two institutions, one, a Public Hospital of the Social Security system with 124 operations and 237 cases at a Private Hospital. At the Public Hospital most of the cases were done following the traditional open repair without mesh, in the Out-Patient Department, under epidural anesthesia. At the private hospital, one third of the cases were done by laparoscopy, with the use of a mesh. The laparoscopic approach was 102% more expensive than the traditional surgery. We conclude that for socioeconomic reason, the patient with inguinal hernia should be operated in the Out-Patient Department under regional anesthesia with the traditional, open technique.
