Effect of body mass index at transplant on kidney transplantation outcomes

ABSTRACT
Background: The relationship between body mass index, patient and graft survival after kidney transplantation is a matter of debate.
Objective: We evaluated the impact of body mass index in renal transplantation.
Methods: A retrospective analysis of 260 kidney transplant patients over 18 years old was performed. Body mass index was categorized as: underweight, normal, overweight and obesity (World Health Organization guidelines). Endpoints were graft/patient survival, delayed graft function, surgical complications and graft function.
Results: There were not differences in demography, medical history and immunosuppression between categories. Wound complications were higher in obese patients (12.5%). Delayed graft function was high in obese patients (25%). Urological complications were higher in overweight patients (15%). There were no significant differences in serum creatinine during five year follow-up. Creatinine clearance in obese patients was significantly higher at one and three years. Lowest 5 year patient survival were in underweight patients (78%) followed by obese patients. 5 year graft survival in patients with normal body mass index, obese and overweight patients was 85%, 80% and 63% respectively.
Conclusion: Body mass index affects transplant outcomes. Obese patients have higher complications, delayed graft function and lower graft survival. Underweight patients were similar predisposed to worse transplant function and graft survival.
