ABSTRACT

The loss of domain in patients with incisional hernias is a problem that sooner or later the surgeon has to face. Due to the lack of general concensus, the denomination of a hernia with loss of domain is so far subjective, and currently its handling is also complicated. The preoperative progressive pneumoperitoneum – initially proposed by the Argentinean surgeon Gońi Ivan Moreno, in 1940, and amended several times – is a method that seeks to increase intra-abdominal pressure and abdominal cavity volume gradually, among other effects, to reduce the content of their continent. We present the case of a 34 year-old female patient with a giant incisional hernia diagnosed and successfully managed with preoperative progressive pneumoperitoneum.
