Local anesthetic thoracoscopy as diagnostic and therapeutic procedure

ABSTRACT

The local anesthetic thoracoscopy (LAT) is a minimally invasive method that allows the direct visualization of the pleural cavity as well as the taking of biopsy samples and the therapeutic interventions in pleura. It is necessary a pre-approach in patients who have showed a pleural effusion through the image and its main cause is unknown or with suspicion of malignancy by observation of a mass in pulmonary parenchyma. The technique is similar to the endopleural tube procedure, performed in an endoscopy room under local anesthesia and/or sedation and consisting of in breathing air into the pleural cavity to provoke an incomplete lung collapse by means of which a trocar with an optical device is introduced, occasionally it is necessary the introduction of a second or third trocar in order to manipulate more instruments. This technique is less invasive, economical and allows diagnosis and management in little time. The procedures via LAT are Pleurodesis, bullectomy, sympathectomy, pericardial fenestration and partial lung recession. Presents a high diagnostic sensibility for multiple pathologies, 92.6% in malignant pleural disease, 90% to mesothelioma and 93.8% in tuberculous pleurities.
