Medical-surgical management of gastroschisis: a multidisciplinary approach

ABSTRACT

Gastroschisis is a severa structural anormaly in the abdominal wall of the fetus, in which there is exposure of intestinal contents into the amniotic fluid, through which loops herniated intestinals, stomach, liver and occasionally, the bladder. The exposed digestive tube undergoes a sustained process of inflammation in the seromuscular coatings resulting in severa serositis witch destruction of the Cajal cells causing hypomotility of the intestine in the postnatal period. There are different theories about the origin of gastroschisis. The most accepted theory at this time is in relation to the early deterioration of the vessels which supply the abdominal wall. The incidence is 1 en 3,000 newborns. The aim of the presentation of this case is to support the new trend in reducing the exposure time of the intestinal loops in the amniotic fluid, performing surgical intervention in newborns affected by gastroschisis in early gestational age of 34-35 weeks; avoiding intestinal edema and inflammation.
