ABSTRACT

Any alteration allowing for urinary stasis and infection can lead to the development of stones.
We present the case of a staghorn stone associated with complete ureteral duplication and emphasize individualized treatment for each patient. Heminephrectomy and right superior ureterectomy were performed on a patient being studied for recurrent pyelonephritis, who had staghorn renal lithiasis associated with ureteral duplication. The procedure was carried out by means of a right paramedian incision with 10 minutes of warm ischemia. A double-J catheter was placed at the lower portion of the ureter. Twelve weeks after surgery, the catheter was removed and the patient was evaluated through excretory urography that showed findings corresponding to postoperative changes. A posterior urine culture was negative and the patient was released from the Service.
Anatomic malformations of the collecting system facilitate urinary stasis, reflux, infection, and in turn, the development of lithiasis. Although there are few cases of lithiasis associated with anatomic malformations, the majority of reports are on non-staghorn stones. The management of these patients depends on each particular clinical situation, and there should be no hesitation in performing opensurgery or renal parenchymal resection when indicated.
